Sample letter of intent

To Whom it may Concern:

I am considering weight loss surgery because I want to improve my health. Over the years I have developed several medical problems related to my weight such as diabetes, high blood pressure and sleep apnea. These conditions have limited my quality of life and may eventually lead to my death. I would like to be able to get off some of my medications and improve my overall life and I feel the surgery will help me achieve this. From the seminar, I learned that the surgery will create a small stomach pouch the size of a thumb. This will restrict the amount of food I am able to eat. I also learned that my intestines will be rearranged in a way that will affect the absorption of nutrients. I know the surgery will require me to make some changes to my current diet like eat more protein, avoid sweets, avoid carbonated and caffeinated beverages and eat an overall healthier diet. I will also need to exercise regularly to make sure I do not regain my weight. I realize the surgery is a tool and not a cure and I will need to make changes to lose weight. My family is very supportive of me having the surgery and they plan to help me through the recovery process.
